Know your rights and responsibilities as a Medical Associates patient.

Your Rights as a Patient of Medical Associates

  1. You have the right to voice complaints or appeals about Medical Associates and the care provided.
  2. You have the right to participate with practitioners in making decisions about your healthcare.
  3. You have the right to be treated with respect and recognition of your dignity and right to privacy.
  4. You have the right to receive information about Medical Associates' services, practitioners providing care, and your rights and responsibilities.
  5. You have the right to a candid discussion of appropriate or medically necessary treatment options for your conditions, regardless of cost or benefit coverage.
  6. You have the right to make recommendations regarding the Medical Associates patients' rights and responsibilities policies.

Your Responsibilities as a Patient of Medical Associates

  1. You are expected to supply information (to the extent possible), that Medical Associates, and its practitioners and providers, need in order to provide care.
  2. You are expected to follow Medical Associates' instructions and guidelines for care that you have agreed on with your practitioners.
  3. You are expected to understand your health problems and participate in developing mutually agreed upon treatment goals to the degree possible.
